Cancellation Policy

ATTUNED — INFORMED CONSENT FORM 1. Nature of Services Attuned is an independent referral and provider-matching platform only. Attuned does not provide therapy, psychotherapy, counseling, medical diagnosis, psychological treatment, or any clinical healthcare services. Our role is strictly limited to facilitating introductions to independent professionals. 2. Professional Credentials & Verification Professionals recommended through Attuned operate independently. Attuned does not warrant, verify, or guarantee local licensure or clinical credentials in every specific jurisdiction. You are solely responsible for independently verifying the credentials, licensing, and suitability of any professional before engaging their services. 3. Not a Substitute for Emergency or Clinical Care Services provided through Attuned referrals are not a substitute for licensed clinical treatment or psychiatric care. Attuned does not handle mental health crises or medical emergencies. If you (or your minor child) are experiencing an immediate crisis or danger, please contact local emergency services immediately. 4. Voluntary Data Submission & Notice of Collection In accordance with Section 11 of the Israeli Privacy Protection Law (5741-1981), you acknowledge that providing personal and health-related information on this intake form is done entirely of your own free choice and is completely voluntary. Information collected will be processed strictly in accordance with our Privacy Policy to facilitate matching and referral services. 5. Cross-Border Services & Data Transfers You acknowledge that independent professionals and cloud storage technology (e.g., Wix, Google) may be located outside your country of residence (including in Israel, the European Union, or the United States). By submitting this form, you voluntarily authorize necessary international data processing to facilitate matching. 6. Consultation Recording (Optional) With your prior explicit consent, Attuned may record your initial matching consultation solely for internal team review to improve referral accuracy. Recordings are stored securely and permanently deleted within 30 days. 7. Right to Withdraw Consent You may withdraw your consent for data processing at any time by contacting Attuned at hello@attunedconnect.com. Withdrawal of consent does not affect processing carried out prior to withdrawal.
